The physicians and hospitals that accept the DeVry Student Insurance (MEGA) are indicated by an astersik(*). Benefit difference for MEGA carriers will vary depending on the plan purchased.

Example: Physician visit is $80. Plan I covers $40 of visit, you pay $40. Plan II covers $50 of visit, you pay $30.

Your physician/hospital can file your claim using HCFA Form 1500 of Hospital Form UB82-UB92. Prescription claim forms are available through your physician/hospital.
